

Community Sign-up Sites
SPONSORED BY THE PRESCRIPTION ASSISTANCE NETWORK OF STARK COUNTY.
For the convenience of the community, the Network Collaborative holds Community Sign-up sites in Alliance, Canton and Massillon. Volunteers and staff will answer questions about prescription programs and services and enroll individuals into programs.
Please call for an
appointment time. Walk-ins are welcome.

Please bring copies of proof of income, your medicine bottles, and physician’s name, address, and phone number, plus the following:
1. If you file income tax, a copy of your most recent federal tax return.
2. If you do not file income tax, a copy of your Social Security Award letter and/or pension statements.
3. If you do not have your awards letter, you may send a copy of your most recent bank statement showing direct deposits of your social security check or pension income.
4. If you are working and your income is from wages earned, please bring one month of recent paycheck stubs.
Other instructions are:
1. You must be a Stark County Resident.
2. You must have no prescription coverage.
3. Federal Income Poverty Guidelines are approximately $20,800.00 per year for an individual and approximately $28,000.00 per year for a couple. The income guidelines are estimates as the individual pharmaceutical companies set their own guidelines and they vary from company to company.
